Supreme Court Rule 3.030  Membership, practice by nonmembers and classes of membership

• Complete the Out-Of-State Certification Request Form and have it notarized.  The form needs to include the Kentucky Court name and full case number for which the request is being made.  Note:  The Kentucky Bar Association does not authorize out of state attorneys for Federal Cases. 
• Return the original form including original signature and notary seal with the per case/per attorney fee of $270 to the Membership Department at the Kentucky Bar Association.  The KBA's address and phone number are supplied on the form.
• The KBA does not accept credit cards for payment of this fee.  Make checks payable to the Kentucky Bar Association.
• A letter/receipt which certifies that the request was made and the fee was received by the KBA will be sent by return mail.  If the letter/receipt needs to be faxed, please enclose any special instructions.  If no special instructions are received, the letter/receipt will be returned to the attorney seeking the certification. 
• It is the responsibility of both the Kentucky co-counsel and the attorney seeking certification to provide the letter/receipt to the court.
